Does the lip go flush with the 1/2" backer board or does it rest on top of the board?

It can be installed flush if you shim the niche to be flush with the backer board and waterproof the joint before installing tile. If installed over the backer board, it creates a slight hump but can be leveled with mortar.

Can this be hung upside down?

Are you able to install tile on this?

Yes, tile installs the same as putting it on the wall. Many customers have successfully installed subway tile and mosaic tile on it.
